Are you concerned that your elderly loved one is having frequent falls, showing signs of forgetfulness and/or not taking medications as recommended?  It may be time to tap the extensive knowledge and services of a case manager.  This service can be provided to the senior whether residing in assisted living, a nursing home or his or her own home.  Families who live a long distance feel greater peace of mind if there is an independent visit done periodically.

ASSESSMENT:

SSN will conduct a needs assessment to identify areas of concern and outline recommendations to make your elder safer and more secure.  During this meeting we will assess the needs of the individual and set up a plan for the appropriate services needed and agreed upon.

IMPLEMENTATION:

Implementation of the plan can be done either by the client/family or by SSN staff.  This would consist of arranging and monitoring selected services along with providing ongoing communication with all parities involved.

FEES FOR SERVICE:

Private Case Management assessment and plan is a fixed fee cost.  Implementation of the plan and monitoring selected services is an hourly charge.
